CARERS

We understand the contribution that Carers are making to the community and we would like to support patients who are Carers

If you are an unpaid Carer who looks after family, friend or a neighbour we would would like you to complete the form below and we can then add this to your record. We are here to support you and if you can only make appointments on certain days we will do all we can to give you an appointment on that day.

There are organisations available to support you if you are an unpaid Carer or know someone who is.

We have a Carers Champion at the practice, Mel Armstrong who is responsible for keeping staff and patients up to date with all services and support available.

Managing malnutrition: advice for carers

    • Malnutrition is a term used to refer to those that have deficiency of energy, protein and other nutrients. Carers play a key role in managing malnutrition as many people they care for are at higher risk. Causes of malnutrition can include:
    • unintentional weight loss
    • poor food intake
    • struggling to swallow food and drink
    • bowel issues
    • Which is why the NELFT have produced this advice for carers. If you think someone in your care may be suffering with malnutrition, keep a weekly food record for them to help monitor their intake. You may also find this guide to making the most of your food intake useful to. For further guidance on how to use these resources see ‘nutritional pack contents page’.

Carers Noticeboard Nov2024.docx

Carers’ Rights Day (21 November 2024) is a national campaign that aims to raise awareness of what rights carers have and signpost to information and guidance.

 

Essex County Council will be sharing communications over our social media platforms to reach as many carers as possible to help raise awareness of carers rights.

 

The council has produced a series of information videos to help carers understand their rights. Full link:

Essex County Council also have resources and tools available for professionals which provide information on the new carers model and the support available to carers.
