Learning Disability Health Check
It's time to book yours.
If you are aged 14 or older and have a learning disability, we'd like to see you for your annual health check.
Patients who have a Learning Disability (LD) are supported by our Doctors and Nurses, our lead LD GP is Dr Shania Khatun. Patients are offered an annual assessment, this is with one of our nurses or a healthcare assistant.
This allows patients or carers to ask any questions about their health, the nurse can refer you to a specialist team if it is needed. You can also have your urine tested/dipped at this appointment.
A general health check will be done to make sure you are getting the best treatment. The easy read leaflet to fill in the questionnaire, and the questionnaire will be sent to you. You will be sent the Health Action Plan so that you can follow to help with your health.
Please ensure you attend the surgery for your health check when you are invited.
